Biography

Ana Tuya Fulton, MD, MBA, FACP, AGSF is an Associate Professor of Medicine and Psychiatry Human Behavior at Brown University, where she also serves as a clinician educator. She is the Chief Population Health Officer at Care New England and concurrently the President and COO of Integra Community Care Network. Dr. Fulton completed her internal medicine residency and chief residency, along with a geriatric fellowship at Brown. Her research interests focus on end-of-life care, improving transitions of care for older adults, and enhancing the quality of care for patients with advanced cognitive impairment through the integration of geriatrics and palliative care across healthcare systems. She has played a pivotal role in the development of Integra @ Home, a hospital-at-home model designed to serve frail older adults and reduce unplanned acute events. Additionally, she is a faculty expert for the Center to Advance Palliative Care's Serious Illness learning community, helping accountable care organizations improve care for seriously ill patients. Dr. Fulton has presented nationally on various topics in her field and has published over 29 peer-reviewed articles. Her work has included significant funding from the Department of Health and Human Services for the Geriatric Workforce Enhancement Program, aimed at enhancing geriatric care across Rhode Island.
