Dr. Mary Naylor

Biography

Mary D. Naylor is the Marian S. Ware Professor of Gerontology and Director of the NewCourtland Center for Transitions in Health at the University of Pennsylvania. She has pioneered the design, evaluation, and spread of healthcare innovations to significantly improve outcomes for chronically ill older adults and their family caregivers, while also reducing healthcare costs. Naylor's work has notably resulted in the Transitional Care Model, an advanced practice nurse-led model that facilitates better transitions for older adults navigating complex healthcare systems. Her extensive research has focused on preventing avoidable hospitalizations and enhancing care in primary settings. Naylor has served as a member of the Medicare Payment Advisory Commission, advising U.S. Congress on policies for Medicare beneficiaries. Through her leadership, her team has advocated for evidence-based changes in healthcare practices and policies internationally. Naylor's impactful contributions have been recognized with multiple awards, including the Gustav O. Lienhard Award for Advancement in Health Care. She is also a Fellow of the National Academy of Medicine and has held prominent advisory roles in various health policy organizations.
